Position Summary
We are seeking a dedicated Long-Term Care Pharmacist to join our pharmacy team. The ideal candidate will be passionate about geriatric care, detail-oriented, and committed to ensuring safe and effective medication use in compliance with state and federal regulations. You will work closely with nurses, physicians, and caregivers to optimize patient outcomes and support facility compliance.
Key Responsibilities
Review and verify prescription orders for accuracy, appropriateness, and compliance with regulations.
Conduct drug regimen reviews and medication therapy management for LTC residents.
Provide clinical consultations and recommendations to healthcare teams.
Ensure compliance with all CMS, state board, and facility requirements.
Counsel facility staff on medication administration, storage, and safety.
Participate in quality assurance programs and medication audits.
Maintain accurate records of all dispensed medications and clinical interventions.
Stay current with developments in geriatric pharmacotherapy and long-term care regulations.
Give vaccination clinics in facilities.

Requirements
Qualifications
Required:
Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy.
Current, unrestricted pharmacist license in Wisconsin.
Knowledge of geriatric pharmacotherapy and LTC regulations (OBRA, CMS, state laws).
Strong attention to detail and ability to work in a fast-paced environment.
Valid certification and CPR training for giving vaccines.
Preferred:
Prior experience in a long-term care or institutional pharmacy setting, but not required.
Certification in Geriatric Pharmacy (BCGP) or Consultant Pharmacy (FASCP) a plus.
Familiarity with pharmacy automation systems (e.g., Parata, TCGRx, QS/1).
